The Service We Provide

Our three-step approach is designed to meet your needs thoughtfully, thoroughly and efficiently:

 1. Assessment:   Identifying Issues and Concerns

First, we meet with you and your loved one to listen to your concerns. We conduct a comprehensive assessment to identify issues preventing your relative from functioning at their best. Particular attention is paid to in-home safety, personal care and household management. We also explore cognitive status, emotional issues and social satisfaction. If you cannot be present, we can review the assessment via telephone.

2. Individualized Care Plan: Developing Step-by-Step Solutions

Next, we develop a comprehensive and individualized care plan based on the needs, circumstances and preferences of you and your loved one. This step-by-step approach helps you move forward with a thoughtful plan to ease and improve care for your aging relative. We identify appropriate referral agencies and the estimated cost for the respective services. We then present these care plan recommendations to you in a meeting, via telephoneor email, whichever works best for you. 

3. Follow-up:   Ensuring the Best Course of Care

We follow-up with you by phone two weeks after we present the care plan to ensure that the approach is well-received. We are also available to monitor the services longer term and respond to your changing circumstances through ongoing reassessment, for an additional hourly fee. 

The Cost of our Service

The total cost of the Assessment, Individualized Care Plan and short term Follow-Up is $350. Extended follow-up is also offered at an additional hourly fee.
